Extra-Mural partners with Mental Health and Family and Community Services (FCS) to provide an assessment, which will determine where an individual’s needs can best be met in the community.

This is an entirely voluntary program. Clients must be willing to be assessed and are involved in approving the plan of care to meet their needs.

Goals of Long Term Care Service

• Allow individuals to have their needs met at home with appropriate enhanced services

• Recommend appropriate placement for individuals, depending on their level of care needed – special care home or nursing home placement

Eligibility Criteria of Long Term Care Service

• Be 19 years of age or older;

• Be a resident of New Brunswick; and

• Be willing to submit a financial statement

• Referral sources can include a self-referral; as well as other sources, such as: family, friends, and physicians
