Interdisciplinary Teamwork Goal

The goal of interdisciplinary teamwork is to provide optimum client focused care through shared goals, cooperation, collaboration and respect among members of the team providing service to clients. The client and family’s goals direct the plan of care as well as the interventions that are provided by the team.

Care and services provided by EMP strive to be client centered – an approach to the provision of services which embraces a philosophy of respect for and partnership with people receiving services, promotes client participation, open exchange of information, client choice in decision making, respect and flexible service delivery is interdisciplinary and provided in a systematic manner that promotes quality.

Each professional on the EMP team draws upon their particular knowledge base in selecting interventions. It could be said that while each discipline has its expertise, EMP professionals are primarily generalists needing to be comfortable in responding to a diversity of issues and concerns.

Treatment settings often include schools, nursing homes, private homes and may also include recreational facilities and outdoors – where ever goals might be accomplished while maintaining safety of patient and the professional. The EMP team needs to be creative, innovative and holistic in their intervention in effort to develop a plan of care which meets unique needs of each client.

The Extra-Mural Program has always employed an interdisciplinary approach to client care, whereby some aspects of the professional’s roles and practices overlap.

The team (including patient and family) define the plan of care. All interventions flow from these collaboratively defined goals.

Teamwork ensures that client care is carried out effectively and efficiently and is promoted by:

  1. The use of team conferences, consultations and interdisciplinary referrals
  2. The uses of client care standards
  3. Utilizing one care plan
  4. Having one set of progress notes
  5. Focusing on team discharge planning
  6. Discipline-specific key indicators that have been developed to assist with the in-office referral process; this focuses on client problems and the discipline(s) most required for service provision.
