Rehabilitation services are provided to both acute and long-term clients and are part of the interdisciplinary care provided by the client care team.

Goal of Rehabilitation Service

The goal of Rehabilitation Services is to prevent the onset of impairment or to limit deterioration in function as a result of impairment.

Eligibility Criteria for Rehabilitation Service

To be eligible for rehabilitation services an individual or group of individuals must:

• Meet the generic eligibility criteria

• Have, (or have the potential for) a functional limitation that can be addressed by at least one of the Extra-Mural Program’s rehabilitative health professionals (Speech Language Pathology, Physiotherapy, Occupational Therapy).

• Referral sources can include self, school personnel, nursing home, family, physician.
