Chronic care implies services for chronic health conditions. Individuals with chronic health conditions may be disabled, experience aging debilities or have a high potential for deterioration or relapse.
Goals of Chronic Care Service

The goals of chronic care are to:

  • Maintain or improve functioning
  • Maintain modified independent living
  • Avoid or delay institutionalization or movement to a higher level of institutionalization.
Eligibility Criteria of Chronic Care Service

To be eligible for chronic care services the client must:

  • Meet the generic eligibility criteria
  • Have a chronic health condition, be disabled and/or experiencing debilities
  • Require home services for an extended period of time
  • Have an attending physician who has admitting privileges to a regional hospital corporation and is agreeable to be the attending physician.
