Acute care services are intended for acute illnesses requiring home care services for a short time. The service may prevent or delay an admission to a medical care institution or facilitate earlier discharge from a medical care institution.

Goal of Acute Care Service

The goal of Acute Care is to achieve the previous level of health and functioning or to provide assistance in realizing a maximum level of health or functioning. Formal support services may be provided to augment the individual's informal support system.

Eligibility Criteria of Acute Care Service

To be eligible for acute care services the client must:

• Meet the generic eligibility criteria
• Require home care services for a short time
• Have an attending physician who has admitting privileges to a regional hospital corporation and is agreeable to be the attending physician.
